Care Navigator

The Care Navigator service is a pioneering project aimed at helping older people stay well and independent in their own homes. The service is free, confidential, impartial and non-judgemental and is designed to make life easier for older people when dealing with problems or changes in their lives.

At the best of times life can be challenging and the Care Navigator can offer clients help and information to guide the way through a variety of issues.

The Care Navigator can provide awareness of community services, help clients take charge of and manage their own affairs, improve decision making ability, increase confidence in dealing with everyday problems and generally improve an individual’s sense of wellbeing.

The Care Navigator works closely with the “Well-being at Home” service.

Case Studies

Miss R is 60 and lives on her own in a council bungalow and uses a mobility scooter to travel around. The scooter is stored in her garden shed and due to the uneven surface around her bungalow, Miss R had found it increasingly difficult to gain access to the shed which meant she was unable to go shopping or visit friends. Unfortunately not being able to get out had left Miss R feeling anxious and depressed.

Miss R contacted the Care Navigator and after discussing various options, she decided that the Citizens Advice Bureau would be the most appropriate organisation to help her communicate with the council. The Care Navigator was also able to put Miss R in contact with Help the Aged who came and fitted grab rails around the property.

Miss R now has greatly improved access around her bungalow and can easily access her mobility scooter. She is able to regularly visit friends and family, do some shopping and attend a local social group. The feelings of depression have lifted and Miss R is far happier and healthier as she is now again able to live her life more fully.

Mr D is 77 and contacted the Care Navigator for advice on benefits. Following discussions, arrangements were made for the Pension Service to come out to see him and assist with the completion of the “Attendance Allowance” form and provide help in applying for claiming the “Disability Allowance”, both of which Mr D is now receiving.

The Care Navigator was contacted by a relative of Mr D who said “ I have just had a lovely phone call from my grandmother telling me you had been to see her, you have made her day. You are the ray of hope we have been looking for. Thank you so much for your time.”
