The Care Navigator & Community Equipment Service:

is aimed at helping older people stay well and independent in their own homes.

is a partnership between Volunteering Ashford, Kent County Council and

East & Coastal Kent Primary Care Trust

The Care Navigator & Community Equipment Assessor can offer you help and information to guide you through a variety of issues and generally improve your sense of wellbeing by,

· Providing awareness of benefits

· Providing awareness of community services

· Develop a Support Plan if you require support at home

· Assess & supply equipment and minor adaptations to your home     

· Helping you take charge of and manage your own affairs

The service is confidential, impartial and non-judgemental and is designed to make life easier for older people when dealing with problems or changes in their lives.

We welcome and will respond to feedback and suggestions from you, about how services could be improved.
